Human resources represent a strategic element in hospital organizations because employee performance directly contributes to service quality and organizational effectiveness. In a complex healthcare environment, effective leadership and organizational communication are considered important factors influencing employee motivation and performance. This study aims to examine the role of leadership and communication in improving employee performance through work motivation at Sentra Medika Cikarang Hospital. This research employed a quantitative approach using an explanatory research design. Data were collected through questionnaires distributed to hospital employees using a five-point Likert scale. The data were analyzed using Partial Least Squares–Structural Equation Modeling (PLS-SEM) with SmartPLS 4.0. The results indicate that leadership has a positive and significant effect on employee performance, while communication does not have a direct significant effect on employee performance. Furthermore, leadership and communication significantly influence work motivation; however, work motivation does not significantly mediate the relationship between leadership and communication and employee performance. These findings indicate that leadership plays a more dominant role in improving employee performance compared with communication and motivation mechanisms. This research contributes to human resource management studies in healthcare organizations by providing empirical evidence regarding the factors influencing employee performance. The findings imply that hospital management should strengthen leadership practices, develop effective communication systems, and consider other organizational factors to improve employee performance.
